It's apart of the map I believe, the person that made the map is an avid 4channer.

Tip: It's an invisible button in the map.

The author kept it under wraps until it got pretty popular, got a group of friends and started annoying people with it. I was with them in that server before they started recording. Afterwards he released a version of the map without the trigger.

You can still overlay a custom spray over everyone's screens on a sv_cheats 1 server. An ideal scenario would involve a cheats server full of mics.

That, or someone figures out how to fool the server into accepting ent_ commands from a client.
